Will Medicare Part
A pay for my stay?
To qualify for Medicare Part A, you must have: (1)
a 3-day in-patient hospital stay, (2) eligible benefit
period, and (3) your physician must certify that
skilled nursing care or skilled rehab services are medically

Can I be involved in the nursing home activities?
We encourage you to be involved in the daily life of
the nursing facility by participating in resident council,
activities and sharing special talents.
We encourage your family to be involved in the daily
life of the nursing facility by participating in family
council, care planning, community events, holiday celebrations
and other special events.
We encourage you to participate in planning for your
care. Your
cooperation is essential if we are to achieve the best
possible results. Our staff members and key professionals
meet regularly to discuss our plan of care including
goals, treatments and results. Please notify
the facility staff or Administrator if you do not understand
a course of action, instruction or what is expected

Can I still vote when I’m in the nursing home?
We encourage you to stay active, vote and
remain a part of the community and be informed about
issues, activities and available community services. [TOP

Can I refuse to take my medicines?
You have the right to refuse any medical treatment and,
if you do so, we will inform you of the possible consequences

Who do I call with questions or concerns?
We encourage you to notify the Facility Supervisor on duty,
the Director of Nursing and/or the Administrator immediately
either orally or in writing any concerns or questions regarding
your care and treatment. We will use prompt efforts to
work towards resolutions to your concerns or grievances.
We encourage you to speak up and make suggestions without
fear of reprisal. We have a policy prohibiting retaliation
for voicing grievances.

Can I bring my TV to have in my room?
Yes. We support a homelike environment. We
request that you be considerate regarding the volume of
your television, radio or other loud noises of any kind
that may disturb other residents. [TOP

Can my family bring me food from home?
All food and beverages brought in the facility from the
outside and those obtained from the vending machines within
the facility, should be in compliance with dietary restrictions
based on physician’s orders. All foods kept
at bedside must be kept in sealed packages or airtight

Can I bring my medications with me from home?
No. Please do not bring any preadmission medications or prescription
drugs with you. You may not keep medications at the
bedside. Only individuals whom the Interdisciplinary Team
has determined as safe to self-administer drugs may keep
those medications secure at the bedside. Whether
or not you may keep non-prescription medications or any
products with warning labels such as “keep out of
the reach of children” at your bedside will be determined
as part of your individualized assessment. Please
give these products to the Director of Nursing for assessment. The
accessibility of the products, and consequences of misuse
to you and other residents who may gain unsupervised access
to your room, must be considered in the decision-making

What items may not be kept in my room?
You may not keep potentially dangerous substances (toxic,
flammable, explosive) including aerosol cans, flammable
liquids, cleaning solvents, rubbing compounds, alcohol,
etc., at your bedside.
You may not use electric blankets, heating pads or extension
cords. Other electrical appliances such as fans,
televisions, Christmas decorations, etc., must be approved
by the Administrator and Maintenance Department before

Does the facility supply the resident’s
clothing?
No. You are responsible for providing your personal
clothing and appropriate footwear. To assure items
are not misplaced, please mark all your personal items
with your name using an indelible marker. [TOP

Does the facility have a bank?
Yes. You can have access to your funds, if they
are deposited with the facility, during the facility's posted banking

Does the facility have a barber or beautician?
Our facility beauty shop is available to serve your needs. The
hours of operation and costs are posted in the beauty shop
and available upon your request. [TOP

Do I have a physician while in the nursing home?
Yes. You have the right to choose your personal
attending physician. Your doctor must be licensed
to practice in your state and must follow the facility’s
policies and procedures when caring for you. Your
attending physician is not an employee, contractor, or
agent of the facility. You or your insurer, including
the Medicare program, is responsible for payment to your
physician. If the physician you choose fails to fulfill
the requirements to practice in the facility, the facility
will notify you and your representative and assist you
in choosing an alternate physician who will provide adequate,
appropriate care and treatment. [TOP

Can I have my meals served in my room?
We encourage all residents to eat in the dining room whenever
possible. Exceptions can be made for bedridden or
critically ill residents and for others with special needs.
